Entry Forms
The RFID Journal Awards were established to recognize companies that distinguish themselves by their successful use of radio frequency identification (RFID).
How to submit your application: There are three ways to submit an application. Each is explained below. Once submitted, entries cannot be amended or added to, so please fill out the form completely.
Deadline: All entries must be completed and submitted no later than 5 pm EST on Jan. 30, 2007.
Three ways to enter:
1. Download the Entry Form in PDF format, which shows you what information is required and has a list of questions that will help you provide the information the judges need to evaluate your application. Then, create your submission in an MS Word document and e-mail it to awards@rfidjournal.com no later than 5 pm EST on Jan. 30, 2007. Be sure to provide all the required information.
2. Download the Entry Form in MS Word format and answer as many of the questions as possible as completely as possible. This will ensure that you provide the information the judges need to evaluate your application. Then, create e-mail the MS Word docment to awards@rfidjournal.com no later than 5 pm EST on Jan. 30, 2007. Be sure to provide all the required information.
3. Fill out the electronic form below and submit your entry automatically. You will receive a confirmation by mail that it has been received.
